Job Summary
Develop and maintain dynamic care plans and schedules for clients while managing care teams to ensure continuity and consistency. Coordinate scheduling for caregivers and lead or coach staff, including teaching new graduate PSWs essential caring elements. Establish and modify client care plans, interact with multidisciplinary teams, and participate in the on-call rotation. This full-time role offers steady weekly hours with health benefits for an RPN with valid CNO registration. Applicants must possess a valid driver's license and 1-2 years of nursing experience. Required Qualifications
- Valid CNO Registration in good standing
- Valid driver's license in good standing
- 1-2 years of Nursing experience (preferred)
