Care Manager for Care Management Services

Care Manager for NextStep Care, a non-profit organization providing home and community-based services, responsible for acting as a liaison between members and service providers to facilitate quality and cost-effective care. You will educate members and families about community resources, maintain regular communication via phone calls and home visits, monitor and adjust care plans, and collaborate with internal teams to ensure continuity of care. You will identify and resolve care plan variances, assess member engagement and behavior, participate in after-hours on-call rotations, contribute to quality assurance and education initiatives, assist with training onboarding of new staff, and promote the organization’s image through servant leadership. Requirements include a Bachelor’s degree in social work or related field with 2 years of experience; experience in social work, HCBS, healthcare or geriatrics preferred. Skills emphasized include effective communication, empathy, collaboration, problem-solving, organization, cultural competence, advocacy, ethical/legal knowledge, and proficiency with common software tools (Outlook, Excel, Word).
