Job Summary
Care Management Extender position providing care management functions, documentation, referrals/linkage, monitoring/follow-up, crisis intervention, and participation in interdisciplinary treatment planning. Responsibilities include coordinating referrals, ensuring signed authorizations for health information, facilitating access to services, and maintaining care management documentation. Requires minimum high school diploma or equivalent with eligible certifications (e.g., Certified Peer Support Specialist, CHW) or related experience; two locations in Henderson, NC area; starting pay at $17.30/hour.
Required Qualifications
- Minimum of a high school diploma or equivalent AND meet one of the following criteria: Certified Peer Support Specialists; Community health workers (NC CHW SCCT); Individuals who served as Community Navigators prior to the implementation of Tailored Plans; Parents or guardians of an individual with an I/DD or a TBI or a behavioral health condition (parent/guardian cannot serve as an extender for their own family member); A person with lived experience with an I/DD or a TBI or a behavioral health condition
- 2 years of paid care management type experience with at least 1 year paid experience at any time with population served
