Job Summary
Care Manager oversees quality and continuity of care across our client base, mentoring and supervising caregivers to deliver concierge-level care. Responsibilities include in-home client assessments, building personalized care plans, matching caregivers to clients, conducting quality-assurance visits, serving as primary contact for families, coordinating with healthcare providers, monitoring client conditions, supporting caregiver onboarding and ongoing training, and assisting with scheduling continuity and on-call coordination.
Required Qualifications
- Prior experience in home care, healthcare, case management, or a related field
- Strong leadership and people skills
- Genuine compassion for seniors and individuals needing care
- Excellent communication and organizational abilities
- Comfort using care management software and standard office tools
- A valid driver's license, reliable transportation, and willingness to travel within the CSRA
