Job Summary
Care Manager role overseeing the quality and continuity of home-care services; mentor and supervise caregivers; conduct in-home assessments; build personalized care plans reflecting client needs and routines; match caregivers to clients; perform quality Assurance visits; serve as primary contact for families with warmth and clarity; monitor client condition and adjust care plans with families and healthcare providers; support caregiver onboarding and training; help maintain scheduling continuity and assist with on-call coordination.
Required Qualifications
- Prior experience in home care, healthcare, case management, or related field
- Strong leadership and people skills
- Genuine compassion for seniors and individuals needing care
- Excellent communication and organizational abilities
- Comfort using care management software and standard office tools
- A valid driver’s license, reliable transportation, and willingness to travel within the CSRA
