Job Summary
Conduct home visits to assess clients and create personalized care plans, then train and support caregivers to meet their loved ones' needs. Monitor and report on health status, coordinating with primary care physicians and community resources while facilitating smooth transitions. Maintain compliance with regulations and document clinical records accurately for morbidity patients. This full-time hybrid role requires frequent travel, including driving several hours daily for 3-5 visits, and demands a valid driver's license. A Bachelor's degree in social work or human services is required, with LSW or LISW preferred.
Required Qualifications
- Bachelor's degree
- LSW or LISW in state of employment
- At least one (1) year of experience working with elders or adults with disabilities
- Ability to travel (by automobile) within assigned geographic area
- Valid driver's license
- Automobile insurance
- Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting
- Training, evaluating, and supervising caregivers
- Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting their care related needs
- Documenting in an accurate and timely manner
- Maintain a clinical record for each morbidity patients
- Monitoring the health status of all members
- Ensure that all needed care is properly delivered
- Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc)
- Additional state-specific qualifications
