Job Summary
Conduct field-based case management for vulnerable populations by identifying eligible individuals for enrollment, engaging members through direct communication and community outreach, and coordinating comprehensive care across physical, behavioral, and social services. Maintain continuous care by linking clients with appropriate resources including housing, employment, and substance use treatment while collaborating with multi-disciplinary teams and justice-involved oversight. Document progress within 24 hours, complete peer reviews, and participate in case conferences and supervision. This role supports CalAIM's Enhanced Care Management initiative within HealthRIGHT 360, a nonprofit dedicated to health equity and transitional support for individuals re-entering the community.
Required Qualifications
- High school diploma or equivalent
- Valid California driver's license
- Access to registered and insured transportation
- Current First Aid and CPR certification
- Must complete a background check and Live Scan
Desired Qualifications
- A minimum of two years' experience in the human service field and/or demonstrated expertise in substance use treatment, relapse prevention, and recovery
- Experience working with clients experiencing acute withdrawal from substances
- Experience delivering evidence-based practices
- Experience providing trauma-informed services
- Knowledge of and experience with providing trauma-informed services
