Job Summary
Lead discharge planning by partnering with patients, families, and providers to coordinate care from admission through community resources. Discuss five key areas for home safety, review medications, and explain the plan of care using plain language and teach-back strategies. Assess patient needs against Medicare-Medicaid guidelines to determine admission appropriateness, observation status, and level of care. Collaborate with insurance companies and social work to negotiate coverage limitations while performing concurrent and retrospective utilization reviews. Serve as a clinical resource for case management, quality improvement, and interdisciplinary teams to maximize patient outcomes.
Required Qualifications
- Associate Degree in Nursing
- Minimum of three (3) years relevant clinical experience including demonstrated use of discharge planning, utilization, and quality management skills
- Chart reviewing-auditing, and able to extract and analyze data
- RN License in Vermont in good standing
- Strong clinical skills
