Job Summary
Care Manager position focusing on depression care within a multidisciplinary community health network. Responsibilities include providing brief evidence-based therapeutic interventions for depression, crisis interventions for suicidal/homicidal ideations or related issues, psychoeducation about medical conditions (notably depression), monitoring patient progress with PHQ-9, conducting psychiatric consultations and follow-ups with PCP, delivering monthly telephonic follow-ups to 75–100 patients, coordinating service plans for patients ineligible for Health Homes, escorting patients to specialty medical and social service appointments, utilizing SBIRT for at-risk substance use, and organizing weekly case conferences for medical providers.
Required Qualifications
- Licensed Master of Social Work.
