Job Summary
Provide brief evidence-based therapeutic interventions for patients with depression; perform crisis interventions for suicidal/homicidal ideations or intents, domestic violence, child abuse, etc.; provide psychoeducation about depression; monitor progress using the PHQ-9; conduct psychiatric consultations and follow-up discussions with PCP; provide monthly telephonic follow-up for a caseload of ~75-100 patients; implement and coordinate service plans for patients not eligible for Health Homes; escort patients to specialty medical and social service appointments when indicated; utilize SBIRT to provide brief interventions and referrals for at-risk substance use; coordinate and schedule weekly case conferences for each medical provider.
Required Qualifications
- Licensed Master of Social Work (LMSW)
- Two years care coordination or related experience preferred
- Bilingual Spanish strongly preferred
