Job Summary
Duties include providing brief evidence-based therapeutic interventions for patients with depression, crisis interventions for suicidal/homicidal ideation or related issues, psychoeducation about medical conditions (especially depression), monitoring patient progress with PHQ-9, conducting psychiatric consultations and regular follow-up with PCP, monthly telephonic follow-ups for a caseload of ~75-100 patients, coordinating service plans for patients not eligible for Health Homes, escorting patients to specialty medical and social service appointments, utilizing SBIRT for brief interventions and referrals, and coordinating weekly case conferences for each medical provider. Strong preference for bilingual Spanish.
