Job Summary
The Care Manager role involves comprehensive case management, including assessment, planning, documentation, and monitoring of clients with behavioral health needs. Required Qualifications
- Associates or bachelor's degree in a human service field with two years MH/SA/DD experience
- Licensed RN with two years MH/SA/DD experience
- Masters w/ licensure or Masters in a human service field with one year MH/SA/DD experience
- Bachelors outside of human service field with 4 years’ MH/SA/DD experience
Desired Qualifications
- Experience in coordinating care for individuals with behavioral health needs
- Experience with interdisciplinary treatment planning
- Knowledge of health promotion education techniques
