Job Summary
Provide case management assessment, person-centered planning and documentation, referral/linkage, and monitoring/follow-up to individuals. Facilitate care management assessment/reassessment, develop care management plans, coordinate referrals to medical, behavioral health, and social services, and monitor progress. Participate in interdisciplinary treatment planning, offer crisis intervention when needed, and ensure signed disclosures and information sharing in accordance with regulations. Build and maintain consumer-centered plans, gather comprehensive information across medical, psychosocial, financial, cultural, and environmental domains, and coordinate with family, providers, and supports to achieve desired outcomes and meet BH quality metrics. Maintain provider communication, support authorization processes for health information release, and assist with transitions across levels of care. Education and experience requirements include a master’s degree with licensure or a bachelor’s/associate degree in a human service field with relevant MH/SA/DD experience, or RN with MH/SA/DD experience, with a focus on delivering coordinated, person-centered care within Medicaid/NC Health Choice contexts and ensuring continuity of care and outcomes reporting.
Required Qualifications
- A associates or bachelor’s degree in a human service field with two years MH/SA/DD experience with the population served
- OR a licensed RN with two years MH/SA/DD experience with the population served
- OR Masters w/ licensure, Masters in a human service field with one year MH/SA/DD experience with the population served
- OR Bachelors outside of human service field w/ 4 years MH/SA/DD experience with the population served
