Job Summary
Care Manager assists individuals in maintaining an interactive process of informed decision-making about Long-Term Services and Supports. Coordinates formal and informal caregivers for clients in person-centered care. Conducts comprehensive person-centered assessments in preferred settings, educates clients on program options and DSS guidelines, completes required documentation, monitors care plans and renewals, conducts interviews with clients and families, and collaborates with an interdisciplinary team. May mentor new staff and participate in on-call services. Requires a BA degree and at least 2 years of direct case management experience. May require reliable transportation and occasional long-distance driving.
Required Qualifications
- Bachelor’s degree in administration, social work, public health, recreation, psychology, counseling, gerontology, or related field required.
- Must have a minimum of two years’ experience in health care or human services. A Bachelor’s degree in Nursing, Health, Social Work, Gerontology or a related field may be substituted for one year of experience.
