Job Summary
Conduct comprehensive home-based assessments using CANS, CRAFTT, and SNCD tools to evaluate youth and families. Initiate care planning activities, including intake meetings and safety/crisis plan development, while collaborating with the family to create an Individualized Care Plan. Serve as the key contact for the youth and family to coordinate services and navigate child/family service systems. Travel throughout the North Shore and northern Essex County to support these efforts. Requires a Bachelor's degree, valid driver's license, and experience in crisis management.
Required Qualifications
- Bachelor's degree in human services or related field
- Valid driver's license and reliable transportation
- Experience navigating child/family service systems
- Experience working with child mental health issues and crisis management
- Monday - Friday, 9:00am-5:00pm
- Based in Danvers, MA
- Travel throughout the North Shore and northern Essex County, including Danvers, Beverly, Gloucester, Rockport, Ipswich, Haverhill, Amesbury, Newburyport, and surrounding communities
