Job Summary
Oversee coordinated care delivery for assigned patients and schedule direct care staff for visits within the electronic medical record. Communicate regularly with patients, nurses, home health aides, and claims teams while advocating for patient needs and ensuring clinical compliance. Identify durable medical equipment requirements, coordinate with DME providers, and assist the authorization team with care starts and renewals. Attend medical appointments to support physicians regarding the Department of Labor Home Health Care program and gather necessary medical records. Limit overtime by ensuring authorized units are staffed appropriately and provide EMR and home orientation training to new staff. Participate in a monthly on-call rotation.
Required Qualifications
- High School Diploma
- At least two (2) years of experience in healthcare; or equivalent combination of education and EEOICPA experience
- At least two (2) years of staff scheduling experience
- Must be at least 21 years of age
- Knowledge of HIPAA regulations and the ability to maintain strict confidentiality of patient information
- Ability to establish interpersonal relationships with people from diverse socioeconomic and cultural backgrounds
- Able to wear personal protective equipment (e.g., face mask, goggles, latex/non-latex gloves) as needed
- Ability to work effectively within role independently and with other team members
- Ability to organize and complete work in a timely manner
- Ability to understand medical/surgical terminology
- Ability to travel up to 80% of time in designated territory
- Valid Driver's License
- Criminal background check clearance

- Sex Offender Registry clearance
- Must be able to work with others at all levels of the Company

- Must be completely honest
- Must be able to participate in an on-call rotation

Desired Qualifications
- Associate's Degree or Bachelor's Degree
- At least two (2) years of Department of Labor / EEOICPA experience
- Home health experience
