Job Summary
Conduct intake, complete comprehensive evaluations of client needs, and prepare detailed plans of care with client consent. Coordinate services by facilitating community resource linkage, monitoring support effectiveness, and maintaining regular contact through home visits for consultation and intervention. Provide outreach and information-referral services to individuals ineligible for case management, transport clients to accommodate community relationships, and offer temporary intervention to ex-clients as needed. Maintain daily service progress notes, billing forms, and HIPAA compliance while meeting monthly productivity requirements of 100 hours. Handle assigned on-call phone and after-hours duties. Requires a valid California driver's license, auto insurance, and CPR certification.
Required Qualifications
- B.A./B.S. degree in human services, psychology or related field
- Valid California driver's license
- Auto insurance
- Current CPR certification
- Proof of education
- Two years' equivalent prior case management experience
