Care Manager

Assess, plan, implement, monitor, and evaluate care options to develop individualized plans for patients across the acute care continuum. Utilize independent judgment to manage services while fostering meaningful relationships with patients, their loved ones, and the team. Maintain integrity, compassion, and accountability throughout patient interactions. Requires NYS RN license, B.S. in Nursing, and 3–5 years of recent med/surg or cardiac experience. PRI and/or case management certification preferred. Located in Roslyn, NY at a facility recognized as a national leader in Cardiology.
