Care Manager, Children & Families

Job Summary

Conduct comprehensive assessments for children and adolescents to identify medical, behavioral, and social needs, then develop individualized Plans of Care in collaboration with families and multidisciplinary teams. Coordinate services across healthcare providers, schools, and community organizations while monitoring progress toward goals and facilitating transitions such as hospital discharges and school changes. Maintain regular contact through home visits, community visits, and telehealth to build trusting relationships and empower families to participate in care planning. Refer members to housing, educational, and behavioral health resources, ensuring timely follow-up on referrals and addressing barriers to care. Document all activities accurately in electronic systems to meet Health Home and Medicaid compliance standards, while intervening during crises and collaborating with emergency services when urgent needs arise.

Required Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, Nursing, Public Health, Education, or a related field
  • Licensed Registered Nurse (RN) with a minimum of two (2) years of relevant experience
  • Master's degree in Social Work, Human Services, Psychology, Nursing, Public Health, Education, or a related field with a minimum of one (1) year of relevant experience
  • Relevant experience working with children, adolescents, and families in healthcare, behavioral health, child welfare, social services, or care/case management
  • Knowledge of child development, family systems, behavioral health conditions, and community resources
  • Strong organizational, communication, and problem-solving skills
  • Ability to effectively manage multiple priorities, deadlines, and member needs
  • Proficiency with electronic health records (EHRs), care management platforms, Microsoft Office, and other technology tools
  • Ability to collaborate effectively with families, providers, schools, community organizations, and multidisciplinary teams
  • Ability to work independently while maintaining accountability and productivity in a remote and field-based environment
  • Licensure in Social Work, Mental Health Counseling, or a related clinical discipline
  • Prior Health Home Care Management experience
  • Experience working with children receiving HCBS, CFTSS, child welfare, foster care, education, developmental disability, or behavioral health services
  • Bilingual or multilingual proficiency
  • Successful completion of all pre-employment and onboarding requirements, including verification of work authorization, education, professional references, and background checks
  • Satisfactory completion of all clearances required for child-serving and Medicaid-funded positions, including criminal background check, Medicaid exclusion screening, Justice Center Staff Exclusion List (SEL) clearance, and Statewide Central Register of Child Abuse and Maltreatment (SCR) clearance
  • Child and Adolescent Needs and Strengths (CANS) certification within the first 30 days of employment, achieving a passing score of 70% or higher
  • Active CANS certification and successful completion of annual recertification requirements in accordance with New York State Health Home standards
