Job Summary
Care Manager position in Chatham County, NC; mobile role delivering proactive intervention and coordination of care to Vaya Health members across multiple service domains (MH/SU/IDD/TBI/physical health) with a focus on assessment, care planning, and linkage to services. Responsibilities include conducting Health Risk Assessments, maintaining HIPAA-compliant records, participating in interdisciplinary care planning, coordinating transitions and diversions from institutional settings, and ensuring a whole-person approach within the member’s community. Requires in-person community contacts and travel, proficiency with care management software/AHR, and collaboration with care teams to address regulatory requirements and member needs. Bachelor's degree and relevant experience in behavioral health and LTSS/HCBS are required; NC state licensure considerations apply for RN-licensed candidates. Strong organizational, communication, and problem-solving skills, and the ability to work within NC Medicaid frameworks are essential.
Required Qualifications
- Bachelor’s degree required (health, psychology, sociology, social work, or related human services field preferred)
- Two (2) years of experience working directly with individuals with behavioral health conditions
- Two (2) years of prior LTSS/HCBS coordination, care delivery monitoring, and care management experience
- Experience with Medicaid-related programs and Tailored Plans is preferred
- North Carolina Qualified Professional eligibility per 10A NCAC 27G.0104
- RN licensure by the North Carolina Board of Nursing if applicable
- Ability to complete required trainings and proficiencies as defined by Vaya
