Job Summary
Conduct family-centered strength-based assessments, planning, linkage, and advocacy for children and families while working with parents, foster parents, caregivers, attorneys, and providers. Develop, monitor, and manage safety plans; identify and develop permanency plans for children in out-of-home care. Assess needs, coordinate service delivery, and ensure referrals are completed. Prepare court reports, testify at hearings regarding reunification or termination of parental rights, and ensure court orders are met. Maintain case documentation in SACWIS and other systems. Conduct face-to-face home visits every 30 days to monitor care and facilitate visitation. Provide support to families for stable placement and timely achievement of permanency. Participate in on-call rotations for nights and weekends. Complete Child Protection Certification training and examination.
Required Qualifications
- Child Protection Certification training and examination
- Attend Child Protection Certification training (pre-service)
- Attain a passing score of 86% or higher on the required examination
- Conduct family function and child safety assessments
- Develop, monitor, and manage required safety plans
- Identify and develop plan to successfully achieve permanency for children placed in out-of-home care
- Assess child and family needs
- Develop service plans
- Coordinate the delivery of services
- Ensure all referrals and linkages to children/parents/caregivers are completed
- Prepare all required court reports
- Submit court reports within the required time frames
- Make recommendations
- Testify at court hearings regarding reunification, permanent guardianship, termination of parental rights, or other case management activities
- Ensure court orders are completed within required time frames
- Develop and maintain case and program documentation according to contract
- Ensure documentation occurs within the SACWIS system and other related systems within required time frames
- Adhere to the local SOC values, goals and priorities for Brevard's children, youth and families
- Utilize a family centered strength based approach to care
- Monitor quality of care by conducting face-to-face home visits with children minimally every 30 days
- Facilitate, monitor, and assess visitation between children, parents, and siblings
- Provide support to children, caregivers and family members to assure stable placement and timely achievement of permanency
- Coordinate cases with supervisor, team members, providers, DCF and attorneys as needed
- Review case information prior to every hearing
- Be prepared to answer any questions from the Court regarding the status of the case, services, and progress towards permanency
- Dress appropriately for Court appearances
- Ensure all court hearings are attended timely
- Embrace and participate in initiatives and programs designed to enhance the System of Care
- Participate in the on-call function on nights and weekends
- Fulfill other incidental or related duties as assigned
- Assist and train others
- Perform duties of higher rated positions from time to time for developmental purposes

- Valid Driver's License
- Clearly and concisely articulate programs, proposals, write policies and respond to program inquiries in a professional manner

- Bachelor's degree in social work or related area of study from an accredited college or university
- Related degrees in Criminology, Psychology, Sociology, Counselling, Special Education, Education, Human Development, Child Development, Family Development, Marriage and Family Therapy

Desired Qualifications
- Master's degree in Social Work or related field of study
- Two years' experience working with at risk children and families
