Care Manager

Develop and facilitate complex care management activities for members with mental and behavioral health needs, including high-acuity cases. Conduct phone and in-home visits to assess member status, evaluate resource availability, and develop personalized care plans. Coordinate between members, families, community resources, and care provider teams to ensure timely access to services. Monitor care plans for treatment side effects and clinical symptoms, recommending adjustments as needed. Collect and document member information to ensure compliance with state, federal, and payer regulators. Provide education on procedures, treatment options, and healthcare benefits to members and families. Travel up to 75% throughout the assigned region in northwest Kansas, with a schedule primarily Monday through Friday.
