Care Manager - CCBHC

As a Care Manager, you will work with adults with mental illness and/or substance use disorders, children with serious emotional disturbance, and veterans, providing community-based services and facilitating service linkage in various health systems. Responsibilities include assisting individuals in accessing medical, behavioral health, housing, and social services, providing training in community resources, accurate documentation, on-call crisis intervention, and collaborating with the Clinical Team. The position requires strong person-centered care skills, knowledge of treatment needs, and advocacy capabilities.
