Care Manager

Assess, plan, implement, monitor, and evaluate individualized care options and services for patients across the acute care continuum using independent judgment. Develop and nurture meaningful relationships with patients, their loved ones, and the care team while maintaining integrity and accountability. Requires a New York State RN license, B.S. in Nursing, and 3-5 years of recent med/surg or cardiac experience. PRI and/or case management certification is preferred. Located in Roslyn, NY at a national leader for Cardiology and Open Heart surgery.
