Job Summary
Advocate for clients by offering resources that align with their needs and coordinate care plan development to establish individualized goals. Facilitate in-person introductions between caregivers and families while collecting input from all professionals to develop a comprehensive care approach. Collaborate with key partner agencies to ensure a successful start of care and complete quarterly survey visits with assigned clients. Identify safety and risk issues in client environments and work with Client Coordinators to ensure care exceeds expectations. Maintain an in-office schedule while traveling to client locations, involving frequent driving and multiple stops throughout the day.
Required Qualifications
- Must have a high level of professionalism and integrity
- Must appropriately handle confidential information and follow HIPAA requirements
- Must be available on weekends as clients' needs come up
- Must be able to read, write, and comprehend English
- Demonstrate active listening skills
- Demonstrate manual dexterity
- Lift or move up to 25 lbs
Desired Qualifications
- Bachelor's degree in a related field preferred
- Minimum 2-4 years relevant work experience preferred
- Passion for coming alongside older adults and supporting their journeys
- Knowledge of the home care industry, senior housing and resources for seniors preferred
- Excellent verbal and written communication with strong interpersonal skills. Be a good listener, articulate, good listener and empathetic
- Strong critical thinking and analytical skills. Detailed oriented
- Ability to identify and implement key opportunities for improvement
- Skilled at quickly developing relationships and loyalty with a client-service oriented approach
- Computer skills: Microsoft office, comfortable in learning client care management software systems
