Job Summary
Develop and implement individualized care plans tailored to the unique needs of each client. Conduct assessments to identify client needs and coordinate appropriate services, while supervising and mentoring staff members to foster a collaborative environment. Collaborate with community resources and stakeholders to enhance service delivery, ensuring the well-being of individuals facing challenges in addiction, mental health, and family dynamics. Maintain accurate documentation of client progress and interventions in accordance with organizational policies.
Required Qualifications
- High school diploma
- Reliable vehicle for travel within area
- Ability to work in a fast paced, multi-task environment with competing priorities
Desired Qualifications
- 2+ years of experience in homecare
- Bachelor's degree
- Office/administrative support skills
