Job Summary
Care Manager at AFNB will develop personalized care plans, provide supervision and guidance to staff, conduct assessments to identify client needs, coordinate services with community resources, and ensure quality care and compliant documentation for individuals facing challenges including addiction, mental health issues, and family dynamics. Must have strong communication skills, administrative capabilities, and reliable vehicle for local travel. Required Qualifications
- 2+ years of experience in homecare preferred.
- High school diploma required; Bachelor’s degree preferred or equivalent work experience.

- Ability to work in a fast paced, multi-task environment with competing priorities
