Job Summary
Develop and implement individualized care plans tailored to the unique needs of each client. Conduct assessments to identify client needs and coordinate appropriate services, while supervising and mentoring staff members to foster professional growth. Collaborate with community resources and stakeholders to enhance service delivery, and maintain accurate documentation of client progress and interventions in accordance with organizational policies. Provide support and guidance to individuals facing challenges including addiction, mental health issues, and family dynamics. This role supports A First Name Basis's mission to reimagine care for seniors and individuals with disabilities across four states, utilizing smart clinical systems to ensure compliance and care quality.
Required Qualifications
- High school diploma
- Reliable vehicle for travel within area
- Ability to work in a fast paced, multi-task environment with competing priorities
Desired Qualifications
- 2+ years of experience in homecare
- Bachelor's degree
- Office/administrative support skills
- Excellent written and verbal communication skills
