CARE MANAGER

Initiate, complete, and monitor comprehensive person-centered care plans for adults and children. Coordinate physical and behavioral health treatment plans with primary care providers while facilitating access to community supports for food, shelter, and employment. Provide high-touch in-person support to ensure medication adherence, offer caregiver counseling, and develop discharge protocols following hospitalizations. Conduct health promotion education on independent living skills and actively participate in team processes to share information with doctors and other stakeholders. Maintain accurate records and solicit consumer input to promote a respectful, ethical environment aligned with VBHCS policies.
