Job Summary
Advocate for members to ensure informed decision-making and appropriate guardianship while scheduling and leading interdisciplinary meetings to develop comprehensive, person-centered Life Plans. Implement, update, and monitor these plans, integrating all necessary supports and maintaining ongoing contact with critical people in a member's life. Ensure timely submission of documentation and assist members with benefits, housing, utilities, and safety issues. Report abuse or neglect immediately. This hybrid role requires required visits with members and families in their home, periodic office trainings, and working from home. Candidates must hold a driver's license and reliable transportation if supporting members outside New York City.
Required Qualifications
- A Bachelor's degree with 2+ years of relevant experience
- A License as a Registered Nurse with 2+ years of relevant experience
- A Master's degree with 1+ year of relevant experience
- Strong communication skills, including verbal and written communication skills
- Strong interpersonal and organizational skills
- Excellent organizational, interpersonal, and verbal and written communication skills
- Required visits with members you support and their families in their home
- Attending periodic trainings and meetings in the office
- Working from home
- Driver's license
- Reliable transportation
- Appropriate insurance
