Job Summary
Advocate for members to ensure informed decision-making and appropriate guardianship while scheduling and leading interdisciplinary meetings to develop comprehensive, person-centered Life Plans. Implement, update, and monitor these plans, ensuring integration of all needed supports including medical, behavioral, and community-based services. Maintain ongoing contact with critical people in a member's life, assist with benefits like Medicaid and Food Stamps, and resolve living problems such as housing or judicial issues. Report abuse or neglect immediately. Submit all documentation timely. Full-time hybrid role with required home visits; bilingual candidates may receive additional compensation.
Required Qualifications
- A Bachelor's degree with 2+ years of relevant experience
- A License as a Registered Nurse with 2+ years of relevant experience
- A Master's degree with 1+ year of relevant experience
- Strong communication skills, including verbal and written communication skills
- Strong interpersonal and organizational skills
- Excellent organizational, interpersonal, and verbal and written communication skills
- Driver's license
- Reliable transportation
- Appropriate insurance
