Job Summary
Develop and implement personalized care plans for clients, coordinating and supervising the delivery of high-quality, compassionate care; maintain accurate client records including intake forms, care plans, and progress reports; communicate effectively with clients, family members, and healthcare professionals to ensure seamless care coordination; collaborate with an interdisciplinary team to address client care issues; provide emotional support to promote independence and overall well-being; participate in ongoing training and professional development to stay current with industry best practices and regulations.
Required Qualifications
- 5+ years of experience in a similar care management or healthcare role
- Excellent communication, problem-solving, and customer service skills
- Ability to self-motivate and work independently, with a strong sense of responsibility and accountability
- Strong analytical and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities
- Ability to work in a fast-paced environment, with a positive and flexible attitude
