Job Summary
Coordinate patient care and services by monitoring, evaluating, and managing assignments in accordance with Life Care standards and applicable laws. Serve as a liaison to external case managers, family members, physicians, and community resources while generating clear written communication and maintaining interdisciplinary relationships. Train patients, families, and providers on care standards, implement departmental policies, and assist in patient evacuations as needed. Maintain confidentiality, adhere to the Code of Conduct, and function productively within a collaborative team environment.
Required Qualifications
- Nursing diploma (associate's or bachelor's degree in nursing)
- Currently licensed/registered in applicable State
- Must maintain an active nurse (LPN) license in good standing throughout employment
- Read, write, speak, and understand the English language
- Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
Desired Qualifications
- One (1) year of clinical experience in post-acute care setting preferred
- Prior case management, utilization review, and discharge planning experience preferred
