Job Summary
Care Manager Assistant - LPN sought to coordinate patient care and services in a post-acute care facility. Responsibilities include coordinating, monitoring, and evaluating patient services, serving as liaison to external case managers, families, physicians, and community resources, implementing and interpreting department programs and standards of practice, and training patients, families, and other care providers. Requires strong organizational skills, ability to make independent decisions, knowledge of laws and guidelines governing post-acute care, proficiency with relevant computer systems, and adherence to Life Care's Code of Conduct and compliance training. Required Qualifications
- Nursing diploma (associate's or bachelor's degree in nursing)
- Currently licensed/registered in applicable State with active LPN license in good standing
