Care Manager Assistant

Assist Care Managers in coordinating patient care, including scheduling appointments, managing medical records, and communicating with healthcare providers. Ensure accurate and timely documentation of patient information, including assessments, progress notes, and care plans. Collaborate with interdisciplinary teams to develop and implement personalized care plans addressing patients' needs and goals. Provide support and guidance to patients and families, addressing concerns and questions, and promoting education and awareness. Maintain accurate records of patient contacts (phone calls, visits, emails) and participate in quality improvement initiatives to identify areas for process enhancements.
