Job Summary
Care Manager to provide individualized support and coordination for clients in home health settings. Develop and implement care plans tailored to client needs, coordinate services and resources to ensure optimal support, maintain accurate and timely documentation, and communicate effectively with clients, families, and providers. Requires experience in care management or related field, strong organizational and communication skills, ability to manage multiple cases, and a BA. Flexible caseload with opportunities to make a meaningful impact in the lives of those we serve.
