Job Summary
Conduct client visits in homes, assisted living facilities, skilled nursing facilities, physician offices, and hospitals to assess physical, cognitive, emotional, and environmental needs. Complete Daily Logs in Excel and MyJunna by end of business day, attaching photographs and documenting all observations, concerns, and follow-up actions. Prepare weekly client schedules in MyJunna and Outlook by Friday at noon, rechecking and adjusting them on Sunday evening. Communicate weekly with families and as needed with physicians, aides, and care team members, documenting all interactions. Identify changes in client condition, safety risks, or care-plan gaps and escalate them promptly to the supervisor. Coordinate services across providers and ensure safe transitions between care settings. Participate in care conferences, family meetings, and case reviews as assigned. Maintain strict compliance with HIPAA and company data policies while managing multiple clients and competing deadlines.
Required Qualifications
- Minimum of 3 years of experience in care management, eldercare, hospice, social work, or complex senior care coordination
- Valid Florida driver's license, reliable personal vehicle, current personal auto insurance, and ability to travel locally between multiple client locations each day throughout Palm Beach County
- Dedicated home office with reliable internet, secure computer, and ability to handle protected health information in full compliance with HIPAA
- Must be able to work in varied home and facility environments that may include stairs, uneven surfaces, and exposure to pets or allergens
- Visual and hearing acuity sufficient to conduct clinical assessments, read medical documentation, and communicate with clients and families
- Must be able to respond quickly and calmly in emergency situations
- Must be able to work in varied home and facility environments that may include stairs, uneven surfaces, and exposure to pets or allergens
- Regularly required to drive throughout Palm Beach County to client locations including private residences, ALFs, SNFs, hospitals, and physician offices
- Regularly required to sit, stand, walk, and use hands to operate a computer, tablet, and mobile device
