Job Summary
Care Manager providing proactive intervention and coordination of care to Vaya Health members across mental health, substance use, developmental disability, traumatic brain injury, physical health, LTSS, and related resource needs. Responsibilities include conducting health risk assessments, assembling and updating Care Plans with a multidisciplinary team, ensuring linkage to services, managing transitions and diversion from institutional settings, coordinating with hospitals and residential facilities, maintaining HIPAA-compliant documentation in the AHR, and supporting crisis planning and ongoing care management. The role requires in-person and travel components, close collaboration with care teams, and adherence to NC Medicaid programs and state-specific requirements. Strong organizational, communication, and problem-solving skills, plus proficiency with Microsoft Office and care-management platforms, are essential.
Required Qualifications
- Bachelor’s degree required in health, psychology, sociology, social work, nursing or related human services field
- Two (2) years of experience working directly with individuals with BH conditions
- Two (2) years of LTSS and/or HCBS coordination or care management experience
- Licensure/certification requirement if RN/licensed nursing
- Must meet North Carolina Qualified Professional criteria (10A NCAC 27G .0104)
- If RN with nursing degree, must be licensed by NC Board of Nursing
- Experience with I/DD or TBI preferred
- Proficiency in care management trainings and competencies including BH/I/DD Tailored Plan eligibility, whole-person health, and transitions
- Bachelor’s degree in nursing with RN licensure may require four years of mental health experience
- Graduate degrees in Human Services may adjust required years of experience
Additional Requirements
- Must live in or near Alamance County, NC or within 40 miles of NC border; NC residency required for the role; travel involved
- Must be willing to travel and perform duties in members' homes and various locations within the service area
