Job Summary
Care Manager for the Adult Foster Care (AFC) program providing biopsychosocial assessments, intake coordination, caregiver training and support, and ongoing care coordination for members in AFC settings. Responsibilities include conducting assessments, submitting referrals and eligibility information, coordinating caregiver intake, ensuring AFC plan of care implementation, conducting on-site visits and annual evaluations, maintaining participant records, and making referrals to other services as needed. Requires strong interpersonal and documentation skills, ability to work as part of a team, travel for community visits, and a Bachelor’s degree in Human Services or related field with MA social work licensure and relevant experience with elderly or disabled adults.
Required Qualifications
- Bachelor’s Degree in Human Services or related field
- MA Social Work license from the MA Board of Registration in social work
- at least 2 years recent experience working with elderly or disabled adults
