Care Manager (Administration)

Create patient care plans and review existing ones. Monitor the Group NextGen clinical task inbox and voicemail daily to retrieve and address messages. Use education materials and brief interventions to engage patients, support behavior change goals, and establish follow-up schedules. Track progress via an electronic care management system and maintain accurate, standardized records. Communicate clearly with patients, primary care providers, and external providers regarding care plan updates and patient progress. Maintain a Florida LPN license and Basic Life Support certification. Monday-Thursday 8:00 am-6:30 pm.
