Job Summary
Provides care management services for the ABI Waiver program, coordinating non-medical home care services and developing individualized care plans in collaboration with clients (18-64, or 65+ if grandfathered) and their families; conducts assessments, identifies supports, assists with applications for programs, documents and monitors care plan implementation, and participates in interdisciplinary team activities and on-call coverage.
Required Qualifications
- Master’s Degree in Social Work with LMSW or LCSW license (in good standing) or Master’s Degree in Human Services, Counseling, Counseling Psychology or Rehabilitation Counseling, or Bachelor’s Degree in Nursing
