Job Summary
Care Manager for Children Health Home: performs comprehensive care management, care coordination and health promotion, develops individualized, patient-centered plans of care to integrate medical, behavioral health, rehabilitative, long-term care and social services; uses Health Information Technology to input and share the plan of care with a multi-disciplinary team; follows up on tests, treatments and referrals; manages timely transitions and outreach to re-engage patients in care; identifies community resources and actively coordinates referrals and services; requires travel to client sites in the Albany/Amsterdam area.
Required Qualifications
- Master’s degree with 1 year of experience OR Bachelor's degree with 2 years of experience
- Minimum of 2 years experience working with children and families
- Must have a valid driver’s license and reliable vehicle
- Must have a current and valid driver's license with acceptable driving history to the Agency. Travel is required.
- Weekly travel to meet with clients up to one hour away from home office (Albany, NY)
