Job Summary
The Children Health Home Care Manager is responsible for core services of health home care management including comprehensive care management, care coordination and health promotion, comprehensive transitional care, youth and family support and referral services, and using technology to link services. Responsibilities include creating and updating individualized, patient-centered plans of care integrating medical, behavioral health, rehabilitative, long-term, and social services; identifying primary care providers and other involved clinicians; engaging enrollees in care and coordinating services; monitoring needs across preventive, medical, and behavioral health domains; using Health Information Technology to input and share information across a multidisciplinary team; following up on tests, treatments, services, and referrals; ensuring timely access to follow-up post-discharge and coordinating transitions; identifying community resources and managing referrals and outreach.
Required Qualifications
- Master’s degree or Bachelor’s degree with required experience or RN with required experience
- 2 years minimum experience working with children and families
- valid driver’s license and reliable vehicle
- travel up to weekly with clients up to one hour away from Albany
- experience in child welfare preferred
- ability to work with diverse populations
