Care Manager 1 - Non-Clinical -Stanly County

Care Manager 1 - Non-Clinical provides statewide care management to support Medicaid enrolled members receiving adoption assistance. Responsibilities include assessing, planning, implementing, coordinating, monitoring, and evaluating options and services to ensure seamless, integrated, and coordinated health care; collaborating with the Primary Care Provider, member, guardian, caregivers, family members, and care-management team; addressing medical, biopsychosocial, behavioral, spiritual, and cultural needs; educating members and families about diagnoses, medications, resources, prevention, and risk factors; setting SMART goals and developing/adjusting care plans; applying trauma-informed care, motivational interviewing, strengths-based, and solution-focused modalities; utilizing EMR systems; facilitating referrals to community services; working with multidisciplinary teams; maintaining documentation per policies; traveling within NC; and attending meetings and trainings. Travel by personal vehicle is required within the assigned area and state. Must reside in NC or within 40 miles of the NC border. Qualifications include a Bachelor's degree in a health/social field or RN licensure, 2 years of relevant experience preferred, CCM certification preferred, and an active driver's license with insurance. The role emphasizes independent work within a remote field-based framework and collaboration with a broad care-management network.
