Job Summary
Supervises discharge planning and social services across inpatient, observation, and emergency department units, serving as the primary point of contact for discharge planning barriers and concerns. Implements care-management strategies in collaboration with facility management; participates in multidisciplinary rounds to ensure active staff participation and effective discharge planning. Provides discharge planning and progression-of-care expertise to the interdisciplinary team, uses dashboards to monitor documentation and regulatory compliance, and facilitates care conferences for complex transitions and placements. Also participates in hospital and medical staff meetings to review high-cost or extended-stay patients and difficult placements, addresses patient/staff/physician/department concerns with timely communications, and participates in hiring, onboarding, and mentoring staff.
Required Qualifications
- Associate's Degree in Nursing (ADN) or Bachelor's degree in Social Work (BSW) required
- Registered Nurse (RN) license required or Social Worker (SW) license/credential
- ACM and CCM preferred
- 1+ year direct care management experience
- 2+ front line nursing experience or 2+ social work experience
- Discharge planning and progression of care expertise to interdisciplinary team
- Ability to participate in hiring, onboarding, educating, evaluating, and mentoring staff
