Job Summary
Supervise and mentor a team of Care Managers through regular individual and team supervision, coaching staff through challenging cases and complex behavioral health scenarios. Conduct case reviews to develop effective interventions and care plans while ensuring compliance with Health Home policies and clinical standards. Participate in community-based client visits throughout Brooklyn and Queens, visiting homes, shelters, and hospitals to advocate for clients and ensure appropriate follow-up after ER visits. Maintain quality documentation in the EHR, manage staff schedules and caseload assignments, and collaborate with community partners including shelters and healthcare providers. This full-time role offers a salary of $65,000–$70,000 for a Bachelor's degree holder with at least three years of case management experience.
Required Qualifications
- Bachelor's degree in Social Work, Psychology, Sociology, Education, Nursing, Human Services, Healthcare Management, or a related field
- Master's degree strongly preferred
- At least 3 years of experience in case management, care coordination, or a related field
- Experience working with individuals with SMI and/or individuals experiencing homelessness or housing instability
- Strong communication, organization, leadership, and problem-solving skills
- Ability to work independently while collaborating effectively with a team
- Comfortable traveling throughout Brooklyn and Queens for client and staff support
Desired Qualifications
- Experience with Health Home Care Management
