Care Management - Social Worker, PRN, Days

Promote patient and family wellness by managing, supporting, and developing comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department environments. Collaborate with service team members, physicians, external agencies, and families to ensure quality of care and effective transitions planning. This PRN role requires a Master's Degree in Social Work and preferred credentials such as ACM, CCM, or LMSW.
