Care Management Social Worker Non-exempt

Provides grief counseling, disease-adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients; conducts holistic discharge planning assessments for inpatient, observation, and/or emergency department patients; reviews medical records and integrates clinical, social, and financial factors into the transition of care plan; develops discharge plans with contingency plans for timely care coordination and placement in post-acute or community care; utilizes technology to ensure continuity of care, complete medical records, and accurate discharge reconciliation; requires a Master’s in Social Work.
