Manage, support, and develop comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department environments. Promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources through collaboration with all service team members, physicians, patients, external agencies, and families. Prioritize quality of care and effective transitions planning while working within the Piedmont Newton Hospital team to deliver award-winning care across Georgia.
