Job Summary
Care Management Extender within the Tailored Care Management team supports members through coordinating healthcare, housing, food assistance, and community resources; conducts high-volume inbound and outbound calls to assess needs, develop and update individualized care plans, connect members to providers and community programs, monitor progress, and ensure continuity of care and follow-up among members, families, and treatment providers. Responsibilities include linking members to medication management, coordinating physical and behavioral health care needs, educating members, building trust with practitioners, and facilitating access to services while maintaining performance metrics and productivity standards.
Required Qualifications
- A high school diploma or equivalent
- Must be 18 years or older
- Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist or a parent or guardian of an individual with a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual or Have two years of paid experience performing the types of functions described in the 'Extender Functions' section, with at least one year of paid experience working directly with the Tailored Care Management eligible population.
